Please DO NOT cancel your phone service or otherwise make changes to your account without first contacting Eclipse Communications to confirm the porting status of your phone number. Cancellation or otherwise changes to your current phone service may result in the permanent loss of your phone number!
LETTER OF AGENCY (LOA)
To maintain compliance with FCC regulations, A Letter of Agency (LOA) must be completed by the end-user and supplied to Eclipse Communications.
Thank you for choosing Eclipse Communications as your network carrier. As you are aware, you may continue to use your existing telephone number with Eclipse Communications. In order to transition your current telephone number to the Eclipse Communications network, Eclipse Communications must work with your previous service provider to ensure that your service is uninterrupted, and where applicable, to ensure that your number is transferred.
Your prior service provider requires this letter as proof that you have explicitly authorized and requested that your service and current telephone number be transferred to another service provider. By filling in all the information requested below and signing and dating this letter, you provide us with the authorization to initiate the process of transferring your service and telephone number to Eclipse Communications. You will then be able to use your old number with the Eclipse Communications network.
Please note that policy and procedures differ between phone providers, and that your current provider may require additional information to process your request. Eclipse Communications will work with your carrier to minimize delays, porting requests generally complete within two to four weeks.
By signing below, I designate (a) Eclipse Communications to transfer my phone number from my current carrier to Eclipse Communications, (b) I authorize Eclipse Communications to transfer my service from my current carrier to Eclipse Communications, (c) I authorize Eclipse Communications to obtain billing information, customer service records, and other information pertaining to transition of my service and phone number from my current carrier to Eclipse Communications, (d) I understand that I may consult with Eclipse Communications as to whether a fee will apply to the porting process, (e) I understand that cancellation of the porting process will result in a fee of $20.00 due to expenses incurred against Eclipse Communications.
Please ensure the following information is completed accurately to prevent possible delays. Discrepancies with this form or your current carrier’s bill may result in delays in the porting process.
